1. What services do you provide?

We are a New York State Licensed Home Care Agency and provide Home Health Aide, Personal Care Aide and Nursing services in Brooklyn, New York, Queens, Staten Island, Bronx and Nassau Counties. We also have a CDPAP Program available.

2. How can I apply to work for your company?

You can either come to one of our 3 locations during working hours or click on the following link to apply.

3. What areas do you service?

We service Brooklyn, New York, Queens, Staten Island, Bronx and Nassau under our licensed agency and all of New York under our CDPAP program.

4. How can I get services for someone?

6. How do I complete a timesheet?

You must have the patient sign the timesheet daily after you complete your services in blue or black ink. Make sure it has all of the client information, your information and the tasks you completed as per the patient plan of care daily. You can contact your supervisor for a sample if needed. Please make sure you primarily use the Electronic Telephony System or your APP from HHAexchange, timesheets are there for supplemental purposes in case you have an issue with using the electronic system.

7. How do I make sure I get paid timely?

The best way to get paid timely is by using the Electronic Telephony System or the HHAExchange APP to clock in and out daily. The app also lets you track your calendar to confirm your clock ins.

8. Where can I access my paystub?

After you get your first paycheck you are emailed a link to Viventium. There you can log in and check your paystub information at any time.

9. How can I report if I want to take vacation days?

Please contact your supervisor at least 2 weeks in advance and please complete a vacation request form.

10. What I am feeling sick who do I report it to after the office is closed?

Our office has a 24 hours on-call system. You can contact us at anytime and a supervisor will attend to your needs.

11. How can I inquire about changing my grandmother’s aide?

Please contact us at anytime we have supervisors available 24 hours 7 days a week.

12. What are the office hours?

We are open Monday – Friday 9am to 5 pm and have an on call system available 24 hours / 7 days a week.

13. My address of phone number changed how can I report that?

Please contact your supervisor and complete a change of information form.
